Did CagriSema Beat Tirzepatide? Why the Dose Comparison Matters
A head-to-head trial appears to answer a simple question: which medicine produces greater weight loss? In REIMAGINE 5, the tirzepatide dose, the study population and the lack of a full peer-reviewed report all change what the headline can legitimately claim.
What CagriSema is
CagriSema is an investigational weekly combination of cagrilintide, a long-acting amylin analogue, and semaglutide, a GLP-1 receptor agonist. The two components are intended to influence satiety, food intake, body weight and glucose control through complementary pathways. It is not approved for public use in Brazil and should not be confused with improvised use of two separate products.
What REIMAGINE 5 found
According to preliminary results released by Novo Nordisk on 21 September 2026, adults with type 2 diabetes inadequately controlled with metformin, with or without an SGLT2 inhibitor, received CagriSema 1 mg/1 mg or tirzepatide 5 mg for 60 weeks. In the efficacy estimand, average weight loss was 12.4% with CagriSema and 9.1% with tirzepatide. The reduction in glycated haemoglobin met the criterion for non-inferiority.
Why the tirzepatide dose matters
The trial used tirzepatide 5 mg, its lowest recommended maintenance dose. It did not compare CagriSema with 10 or 15 mg. The accurate conclusion is that CagriSema was superior to tirzepatide 5 mg in this protocol and population—not that it has been proven superior to every tirzepatide dose or for every patient.
What REDEFINE 9 adds
In REDEFINE 9, adults with overweight or obesity and without diabetes received CagriSema 1 mg/1 mg or placebo for 68 weeks. The company reported an estimated average weight reduction of 21% with treatment and 2% with placebo among adherent participants. These figures should not be directly compared with REIMAGINE 5 because diabetes status, background treatment, duration and comparator differed.
Why the result is still preliminary
The manufacturer announced the numbers before publication of a complete peer-reviewed report. Independent assessment still requires participant flow, attrition, full statistical analyses and detailed adverse-event data. Earlier trials support the combination's potential but do not replace the complete reports for these studies.
What the trial does not authorise
The announcement does not make CagriSema available, predict its price or approval in Brazil, or justify seeking cagrilintide or compounded combinations online. It also does not identify the best treatment for an individual. Diagnosis, previous response, adverse effects, contraindications, cost and capacity for follow-up remain relevant.
Why nutrition care still matters
A greater reduction in appetite does not remove the need to organise food intake. Protein and micronutrient intake, hydration, fibre, gastrointestinal tolerance, lean mass and daily routines require attention. The clinical aim is not merely to maximise a percentage on the scale but to protect health and function throughout treatment.
Personalized care
A plan should fit the person, not the trend.
